Step 1
Step 1 - Click the button below to download your quote for your insurance company. You need to send this to your insurance company with your other documentation in order to receive their Approval letter.
Step 2
Step 2 - AFTER you have received your approval letter from your insurer / funding agency, please complete the membership request form below. Please note the request will only be processed if you provide the following the Third party provider Name, Case Manager with email address and Case/Claim Number
Step 3
Step 3 - AFTER we receive payment from your third party provider, a member of our team will get in touch to get you started on your membership journey (please note that third parties are currently taking between 7-45 days to make payment).
What Happens Next?
Once we've received your approval letter and payment from your insurer / funding agency, we'll be in touch with you in regards to:
- establishing your membership with MAC,
- getting your account set up on Active World,
- logging in and using the Active World app,
- booking you in for your first BEGIN appointment
BEGIN with Monbulk Aquatic Centre
Every new member at Monbulk Aquatic is entitled to three (3) FREE BEGIN sessions. These are sessions designed to get people started with their fitness goals.
Beginning Consultation - (Day 1-3)
- Pre-Exercise Screening
- Goal setting
- Program design and demonstration
Check in – (Day 14-17)
- Review Program, modify as required
- Review experience at facility
Benefits consultation – (Day 30-35)
- Member results are measured from initial appointment
- Set plan for following 30 days (including an additional check in if required)

The Active World app allows you to link your family memberships to manage from the once account and will give you greater visibility of your progress and achievements when visiting us!

GET STARTED AS AN EXISITING MEMBER
As an existing member, we have set you up using the email you provided us upon joining – all you have to do is reset your password!
If you are currently using Active World for your GOswim lessons – all you need to do is update the app!
1. Download the Active World App
2. Use the facility code MONBULK
3. Select forgot password to reset your account – follow the prompts
4. Login once ready and create your quick access code – 4 digits
5. Now you're in! Check your programs or book into a new class!
